In regards to glasses, it should be possible, but the legs may scrape the rim of the mask. Also, I would recommend getting lenses that have had anti-fogging treatment if you have the option for that. (I do have to wear mine all the time.)

Typically they are installed in the front-left and front-right above the eyelets in the forehead area. Something worth considering though is to not to have the fans directly aimed in the direction of your real eyes, as that can cause an unpleasant dry sensation.

Consider giving 42agent a look at.

There aren't really any "vetted" outfit makers. But one I can suggest is CosplayFu. There are others like them out there, but some may not offer the same options than others. If you could specify a budget (in USD), you may get better answers. But, it all boils down to the design complexity and fabrics used.

From what I'm seeing, they are a reseller; so they will be certainly selling other makers' products but also as a marked-up price (typically but not limited to shipping costs). What I would do is to look for any logos and search for the maker by name; might save a bit of money to go to the maker as direct as possible.
